Only send notifications to user that are currently assigned or CC'd on a record.

Current behavior is:

* Record authors are getting notifications regardless. Changing the author to a different user will not stop notifications to the original author. See (case #02802405)
* Users that were either assigned to a record or CC'd will receive notifications even if their assignment or CC was removed.

It is not clear why there is no control over stopping notifications from being sent per above. There is clearly a way to control users to start getting notifications.

Please fix - it is driving everyone crazy.
