Improve PCO Markups

A few suggestions to improve PCO markups:

1. On the form, keep markups separate from regular line items, similar to how tax is separated on POs.
2. Allow for default percentages, budget codes, & descriptions, but give the user the ability to change percentages/levels on the fly without having to recalculate them.
3. Figure out a way to allow for "sliding scale" markups. On many jobs, there is not a set percentage for markups - the percentage is determined by the cost of the change.

a. The specfications read something like, "Allowable percentages for overhead and profit on changes shall not exceed 12% on the first $10,000, 10% on the next $10,000, and 6% on the balance over $20,000.
