Add option to Show/Hide - Pass | Attn Req'd | N/A for fields

What are we trying to accomplish?
Simplify checklist forms. Sometimes, we don't need to use these options.

How is the lack of functionality affecting the business?
It makes checklists a little more confusing for some users.

Suggested implementation:

When adding fields to a checklist in the template creation tool, add a checkbox to show/hide these options on the checklist. If hiding them, the end user should not see them at all when they fill out a checklist.
