Ability to Add Two Types of Data Fields for Checklist Templates

I'm trying to create a management walk checklist with dates and building numbers, but each of our apartment projects have different building numbers. If I select a Date field, the building numbers are static. If I select a Text field, we can enter the building number but still missing the dates.
