Add Form Banner Headers to Nomenclature Per Record

We modify quite a few of the field names on a Potential Change Order, Subcontract Change Order to match our terminology. The banner headers, on a PCO for an example, are hard-coded with "Estimate, Proposed, Approved and Applied" which we cannot change. This causes confusion if we can Estimates "ROM" and they can't find the total ROM per PCO.
