Inspection widget - Critical Need!

We need an inspection tool with the ability to create custom inspection forms / check off / pass, fail, NA options. When a fail occurs it triggers an automatic action item or issue item that has to be filled out with the ability to take pictures and reference drawings sections/pages and we need to be able to print these out with our company logo on them in sequential order with the photos and drawings snip in the pdf.
