Please only link 1 drawing when first creating record then adding record link to that drawing added

This is causing quite confusion as it looks like there are 2 different drawings and revisions added when you first create the record (RFI, Submittal, Punch), then open the drawing and link the new record to it.

When you open the record there are 2 different posts, and when you click on the links, there are 2 separate drawings listed under Design and they have different names when they are the exact same drawing and revision One lists the revision number and one is called Current.
